Ive heard that "make out" means to manage to see somebody/something or read or hear something.
but I cant understand it clearly, could you tell me the meaning of it and how and when do you use this phrase?

Think of it as trying to figure out something from limited information.

Well, this phrase has two meanings that I can remember, both of which are colloquial. To be able to make out something means that you could hear or see something, but not entirely. When people say this, they're trying to give the information that they have, but know it may not be entirely accurate.
例: I couldn't really hear them well, but from what I could make out, he's going to go with her tomorrow.
Now as for the other meaning, "make out" means to passionately kiss.

It doesn't mean assume or guess (although assuming and guessing may follow), it means trying to get information and just getting some.
nebulahime14's example is a good one.
'...I couldn't really hear them well...' (I heard them but not clearly)
'...but from what I could make out...'
(...from the little I could hear...)
and then there is an assumption, in that example.
Can you make out this handwriting? I can barely read it. (Can you read this from the small part that might be readable?)

"Make out" has two meanings.
The meaning that you're looking for means:
'to understand.'
Example Sentence: I can't make out what you're saying.
(Means: I can't understand what you're saying. I can't figure out what you're saying.)
Other meaning:
'to kiss... a lot.'
Example Sentence: Harry and Katy made out under the bleachers yesterday.
